GENERAL POLICIES:

This site is not meant to diagnose SIBO. Treatment for SIBO should be done by a qualified and licensed health care provider. As such, kit sales and analysis are restricted to qualified health care providers only.

All SIBO Kit sales are final. Billing for purchasing testing kits will take place after the order has been submitted and prior to shipping. The invoice and receipt will be sent along with shipment. Billing for analysis will take place after the analysis is complete. The invoice and receipt will be sent electronically.?

Sample validity is guaranteed for 14 days following collection. It is strongly recommended that samples be shipped to SIBO Diagnostics as soon as possible following collection and that a shipping method be used that allows for tracking and guaranteed delivery date. Analysis is done in the order in which the completed kits were received.

SHIPPING POLICY:

All SIBO kits will be sent within 3 business days of the order being placed by a courier to the account address on file. The freight cost is billed to SIBO Diagnostics, and is charged to the practitioners office, if the minimum kit order has not been reached. SIBO Diagnostics reserves the right to ship with any freight carrier or speed of service, and is subject to change at the discretion of management. If expedited shipping service is requested, the cost of the shipment will be at the responsibility of the customer.

In order to qualify for free shipping, a minimum kit order must be made:

• 3 Kit Minimum order for Western Canada, including British Columbia, Alberta, Saskatchewan and Manitoba
• 6 Kit Minimum Order for Eastern Canada, including Ontario, Quebec, the Maritime provinces.

The cost of return shipping of the samples is not included, and is the responsibility of the health care provider.
